The employer provided Moldex N95 2300 Series (Size M/L) particulate respirators, Honeywell 7700-30L (P/N 7700-11L) (Size Large), and 3M Half Facepiece Series 6200 (Size medium) tight fitting respirators for voluntary use. The employer failed to provide employees with information contained in Appendix D.
