Citation text line OSH ACT of 1970 Section (5)(a)(1):Section 5(a)(1) of the Occupational Safety and Health Act: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees: crushing hazards due to the improper use of an automotive lift. On or about: July 22 and July 23, 2021 Location: Central Auto Body Collision of Westchester, 422 Saw Mill River Rd. Yonkers, New York 10701 a. Employees were exposed to crushing hazards when using a pneumatic Rakjak DBT2 jack while performing repairs on automobiles. The employer instructed employees to utilize a Rakjak DBT2 pneumatic jack with an operating lever that was not protected from inadvertent activation thereby exposing employees to crushing injuries while performing work under the frame of a vehicle. On or about July 23, 2021, an employee sustained fatal crushing injuries after he inadvertently activated the Rakjak DBT2 while performing work under the frame of a vehicle.
