Material stored in tiers were not stacked, blocked, and interlocked so that they are stable and secure against sliding or tipping. a) At the workplace, in the yard, on or about 10/24/14: Employees were loading 4,000 lb. panels of pre-cast concrete onto a drop-bed trailer where the load was able to be released from the control connection without ensuring that it was stable and secure. Abatement documentation must be submitted for this item. Or, in the alternative: OSH Act of 1970 Section 5 (a)(1) of the Occupational Safety and Health Act of 1970; The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to a struck-by hazard. a) At the workplace, in the yard, on or about 10/24/14: Employees were loading 4,000 lb. panels of pre-cast concrete onto a drop bed trailer where the load was not secure or otherwise prevented from tipping, and where employees following established procedures were exposed to the fall shadow of the loads. Feasible and acceptable methods to correct the hazard include but are not limited to: 1) Modify the base of the "A" frame support so that it meets the slope of the support at a right angle which ensures that the panels are seated correctly on the truck bed. 2) Modify the release procedure and tools to allow the employee to be outside the "fall shadow" of the item being loaded. 3) Follow administrative controls as detailed in the National Precast Concrete Association "Guide to Plant Safety" and "Material Handling Safety in a Precast Plant".
