P & H hoist was not provided with a safety latch on the lifting hook and the beam it was attached to had no rated load markings. b) Plant 1, Press Alley, on or about 8/5/14: A beam with a one ton chain hoist attached had no rated load markings. c) Plant 2, White Bone Dept., on or about 8/5/14: Two beams each with a 1/2 ton electric hoist attached had no rated load markings. Among other methods, several feasible and acceptable abatement methods to correct these hazards are to: 1. Install new safety latches on the lifting hooks in accordance with ANSI B30.16 Section 16-1.2.9. 2. Mark the appropriate rated load markings on the beams. Abatement certification must be submitted for these items. |
