ASC WILL PROVIDE A RANGE OF STRATEGIES AND ACTIVITIES TO ACHIEVE THE PROJECT PERIOD OUTCOMES. ASC HAS ADAPTED PROGRAMMING AND PROTOCOLS TO ENSURE SAFETY OF CLIENTS AND STAFF DURING THE COVID-19 PANDEMIC, AND WILL INCORPORATE THOSE PROTOCOLS INTO THE PROPOSED PROGRAM AS APPROPRIATE. ASC?S COMPREHENSIVE HIV PREVENTION CORE PROGRAM WILL CONSIST OF THE FOLLOWING ACTIVITIES: DIAGNOSE: TARGETED HIV TESTING FOR 400 CLIENTS ANNUALLY, PROVIDE INTEGRATED SCREENINGS FOR 200 CLIENTS ANNUALLY, CONDUCT 10 LARGE SCALE TESTING EVEN(S); TREAT: LINKAGE TO HIV MEDICAL CARE FOR AT LEAST 90% OF NEWLY DIAGNOSED HIV PERSONS AND THOSE NOT IN CARE, RE-ENGAGEMENT, PARTNER SERVICES REFERRAL FOR 100% OF NEWLY DIAGNOSED PERSONS, MEDICATION ADHERENCE PROGRAMMING; PREVENT: REFERRALS TO PREP AND NPEP FOR 95% OF CLIENTS, PROVISION OF INDIVIDUAL HEALTH EDUCATION/RISK REDUCTION, CONDOM DISTRIBUTION, REACHING 95 PERSONS FROM THE TARGET POPULATION ANNUALLY; RESPOND: SUPPORT FOR HEALTH DEPARTMENT CLUSTER DETECTION AND RESPONSE ACTIVITIES. ASC?S OPERATIONAL PROGRAM WILL CONSIST OF PROGRAM PROMOTION, OUTREACH, AND RECRUITMENT; COMMUNITY ADVISORY BOARD (CAB); AND HIV PLANNING GROUP (HPG). THE SPECIFIC SERVICE AREA IN WHICH ASC PLANS TO DELIVER ITS PROGRAM IS NEW YORK CITY (NYC), WITH A FOCUS ON AREAS WITH LARGE IMMIGRANT POPULATIONS, INCLUDING UPPER MANHATTAN, SOUTH BRONX, CENTRAL BROOKLYN, AND STATEN ISLAND. THE SERVICE AREA IS DISPROPORTIONATELY AFFECTED BY HIV AND IS WHERE PEOPLE WITH AND AT GREATEST RISK FOR HIV RES IDE AND FREQUENT. ASC?S TARGET POPULATION FOR SERVICE DELIVERY IS HIGH-RISK HETEROSEXUAL MEN AND WOMEN, AGES 13 TO 64, WITH A FOCUS ON BLACK AND LATINO IMMIGRANTS (PRIMARILY AFRICAN AND CARIBBEAN), INCLUDING WOMEN INVOLVED IN SEX WORK, WOMEN AND YOUNG WOMEN WITH A HISTORY OF TRAUMA, SEXUAL, EMOTIONAL, OR PHYSICAL ABUSE, AND WOMEN AND YOUNG WOMEN IN SEXUAL RELATIONSHIPS WITH PARTNERS WHOSE STATUS IS UNKNOWN OR ARE IN SERO-DISCORDANT RELATIONSHIPS. THE AGENCY IS TARGETING THIS POPULATION AND SERVICE AREA BECAUSE RATES OF HIV/AIDS ARE AMONG THE HIGHEST IN THE CITY. ASC WILL COLLABORATE WITH OTHER ORGANIZATIONS THAT HAVE AN ESTABLISHED HISTORY OF WORKING WITH AND RECRUITING MEMBERS OF THE TARGET POPULATION AT GREATEST RISK FOR HIV ACQUISITION OR TRANSMISSION. ASC HAS DEVELOPED RELATIONSHIPS AND CONNECTIONS WITH A RANGE OF ORGANIZATIONS, INSTITUTIONS, AGENCIES AND BUSINESSES THAT ASC WILL BE ABLE TO LEVERAGE FOR THE PROPOSED WORK.
