Citation text line 29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s) a) On or about 07/14/14, on the large flat roof of Gates-Chili Middle School, 2 Spartan Way, Rochester, NY: Employees were tearing off the old roof in Area 18 at the east end of the roof and transporting the debris via 4-wheel push carts to the dump area at the west edge of the roof which was a distance of approx.. 250 ft. diagonally across the roof. The employees were exposed to a potential hazards such as poor illumination, falls of approx. 12 ft. from roof edges at the courtyard area southwest of the tearoff area, falls from the west edge of the roof in/near the dumpster area, falls from the ends of a temporary ramp, and the structural integrity of that ramp, and falls into a dumpster. Frequent and regular inspections of this jobsite, materials, and equipment were not conducted to ensure that proper illumination and adequate fall protection systems were being utilized. Citation text line 29 CFR 1926.56(a): While work was in progress, construction areas, ramps, runways, corridors, offices, shops or storage areas were not lighted to the minimum illumination intensities as listed in Table D-3 of 29 CFR 1926.56:(a): a) On or about 07/14/14, on the large flat roof of Gates-Chili Middle School, 2 Spartan Way, Rochester, NY: Employees were tearing off the old roof in Area 18 at the east end of the roof and transporting the debris via 4-wheel push carts to the dump area at the west edge of the roof which was a distance of approx.. 250 ft. diagonally across the roof. This activity was being conducted outdoors in the hours of darkness. Employees disposing of the roof tearoff debris were exposed to a potential fall of approx. 12 ft. from the west edge of the roof in/near the dumpster area, and up to a 13 ½ ft. fall from the south and north edges of a temporary plywood ramp installed to bridge the horizontal gap between the roof edge and the dumpster. The only illumination in this area was a double light stand and the faint lights in the parking lot. One of the 2 lights in the double light stand was not working. The lighting in this area did not meet the minimum illumination intensity in Table D-3, which requires a minimum intensity of 5 foot-candles for General construction area lighting. Citation text line 29 CFR 1926.501(b)(6): Each employee on ramps, runways, and other walkways was not protected from falling 6 feet (1.8 m) or more to lower levels by guardrail systems: a) On or about 07/14/14, on the large flat roof of Gates-Chili Middle School, 2 Spartan Way, Rochester, NY: A Dumpster trailer was parked on the ground at the west part of the roof just south of the main entrance. The trailer was approx.. 43 ft. long and was parked such that there was a horizontal gap between the roof and the dumpster of approx. 1 ft. 1 in. at the north end of the dumpster , and a gap of approx. 3 foot at the south end. The height of the dumpster was about 1 ½ ft. higher than the roof. About 5 or 6 sheets of plywood (4x8x1/4) were laid lengthwise to form a ramp which covered about 30 ft. of the 43 ft. long gap between the roof & the dumpster. Employees unloaded the roof tearoff debris from the 4-wheel carts, walked on the ramp and threw the debris into the dumpster. Many times the carts were also rolled up onto the ramp and unloaded. The employees were exposed to a potential fall of approx. 12 ft. to 13 ½ ft. from the north & south edges of the ramp, and a fall of up to about 9 ft. into the dumpster. One employee was seriously injured when he fell off the south end of the plywood ramp. No guardrail system or other fall protection system was utilized. NO ABATEMENT CERTIFICATION REQUIRED

There were partial warning lines in some areas but they were not continuous and there was no monitor(s). Two short sections of a portable guardrail system (shown in police photos) which may or may not have been present at the time an employee fell, did not meet fall protection requirements. No adequate fall protection system was utilized. NO ABATEMENT CERTIFICATION REQUIRED
