No Guardrails were installed. b) On or about 8/15/13, in the New Stair Tower area, of an old two story factory building being converted into apartments at 320 Grote St., Buffalo, NY: Employees of E.B. Atlas Steel Corp. and of other employers were exposed to a fall of approx. 14 ft. from the 4 plank wide second level of a masonry scaffold that was flush with the metal decked canopy. This area was accessible from the second story hallway. No guardrails or other fall protection system was being utilized to protect employees. NO ABATEMENT CERTIFICATION REQUIRED
