Mill Area. On or about 10/20/21, an Authorized Employee was performing maintenance activities on a Unimak Gold 95 Molder (100 psi). The employer did not develop, document and utilize energy control procedures for the pneumatic source of energy for machines including, but not limited to the Unimak Gold 95 Molding Machine. b) Timeless Frames, 1 Fisher Circle, Watertown, NY. Mill Area. On or about 10/20/21, An Authorized Employee performing maintenance on the Unimak Gold 95 Molder (440 volt) did not utilize the procedures for the control of potentially hazardous energy (electrical). The pneumatic energy source was not locked or tagged out prior to beginning maintenance activities on the machine.
