Citation text line 29 CFR 1910.178(o)(1): The employer did not ensure that only stable or safely arranged loads were handled: a) On or about 03/28/2024, in the loading area of the facility; employees were exposed to struck by hazards when loads were not stable and secured to forklift trucks. An employee was seriously injured when an unstable chip conveyor was being unloaded from a flatbed truck. The chip conveyor was picked up from the trailer of a truck with a forklift. When backing up the forklift, away from the trailer, an employee was positioned along side of the conveyor and directing the forklift operator. The coolant in the pan of the conveyor shifted, causing the weight of the load to shift, become unstable and fell off the forks hitting the employee. NO ABATEMENT DOCUMENTATION REQUIRED
