AT THE END OF THE THREE-YEAR GRANT, 423 UNDUPLICATED VOLUNTEERS WILL SUPPORT THE PRIMARY FOCUS AREA OF HEALTHY FUTURES AND WILL IMPROVE HEALTH OUTCOMES FOR 5,943 PEOPLE: 2,628 PEOPLE WILL HAVE INCREASED SOCIAL SUPPORT OR IMPROVED CAPACITY FOR INDEPENDENT LIVING; 2,880 WILL REPORT IMPROVED ACCESS TO MEDICAL CARE, AND 435 PEOPLE WILL REPORT IMPROVED HEALTH. THE CNCS FEDERAL INVESTMENT OF $111,467 WILL BE SUPPLEMENTED BY SECURING FUNDING SOURCES PROVIDED ANNUALLY FROM COUNTY, STATE AND LOCAL AGENCIES. $175,644 (FEDERAL PASS THRU) $22,899 (NON-FEDERAL) FROM THE SUFFOLK COUNTY OFFICE FOR THE AGING, $5,000 FROM THE TOWN OF EAST HAMPTON, $6,014 FROM THE NEW YORK OFFICE FOR THE AGING AND $38,213 FROM FUNDRAISING EFFORTS. THE TOWN OF EAST HAMPTON GIVES A $12,800 IN-KIND DONATION FOR THE USE OF OFFICE SPACE.
