29 CFR 1926.451(b)(5)(i): The end of a platform 10 feet or less in length extended over its support more than 12 inches and the platform was not designed and installed so that the cantilevered portion of the platform is able to support employees and/or materials without tipping, or has guardrails which block employee access to the cantilevered end: a) On or about 3/7/18, at the two-story building area of the site, North Tonawanda, NY. Employee, worked from a scaffold platform made of 2X8 lumber; with one end sitting on the rung of a step ladder and the other end sitting on a bracket made of 2X4 lumber, lacked guardrails on the bracket side, where the platform extended 18 inches over its support, to block employee from accessing the cantilever end.
