Citation text line 29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) MCAD Area, on or about 3/19/18: Denison Multipress was not provided with guarding to prevent employees from having any part of their body in the danger zone during the operating cycle. b) Machine Shop, on or about 4/10/18: Single spindle drill press used for secondary operations was used without a guard. Employees exposed to contact with the rotating chuck and tool during operation. c) ROG Assembly, on or about 4/10/18: Bridgeport Milling machine was not equipped with guard to prevent contact with the rotating chuck and tool. d) Maintenance, on or about 4/10/18: Ridgid pipe threader activated by a manually operated on/off switch that would not automatically shut the machine off should the operator be inadvertently caught up in the rotating parts. e) Aerospace, on or about 4/11/18: Pressure Lubrication Rack was not provided with guarding to prevent the operator from nip point hazards when the hose fixture is cycled back to the starting position. f) Machine Shop, on or about 4/10/18: Bench grinder, Model 09012162, was missing the wheel side guard, thereby exposing the operator to the rotating end nut. g) Stress Relief Area, on or about 4/10/18: Stress Relief Machines #1, #3, #4, #5 and #11, had unguarded rollers at various locations which exposed employees to in-running nip point hazards.

The operator hand feeds and hold 7/8-inch diameter material into the point of operation and cycles the machine using a foot pedal. b) MCAD, Work Center 5, on or about 4/3/18: Ring Swager Crimper, ID No. 444, was not provided with adequate point of operation guarding in that the operator could reach through the guard and into the point of operation. c) Hydraulic Hose Cell, on or about 4/11/18: Hydraulic Hose Vulconizer was not provided with adequate point of operation guarding in that it did not prevent the operator from having their hands in the danger zone during the operating cycle. The operator hand feeds and holds 1-inch diameter hose material into the point of operation and cycles the machine with a foot pedal. d) Aerospace, on or about 4/11/18: Cut-off saw used to cut 1/2-inch to 7/8-inch diameter braided casing was not provided with adequate point of operation guarding and did not prevent employees from having any part of their hands in the danger zone during the operating cycle.
