The employee could not recognize hazards associated with scaffolds, and was exposed to hazards, including but not limited to: employee exposed to fall hazard, and employee worked on scaffold platform not fully planked or decked, employee has no safe access to scaffold platforms, and scaffold legs and frames not bear on adequate firm foundation. On or about 03/28/12. NOTE: IN ADDITION TO CERTIFICATION OF ABATEMENT, THE EMPLOYER IS REQUIRED TO SUBMIT DOCUMENTATION OF ABATEMENT FOR THIS ITEM.
