29 CFR 1926.451(e)(1): When scaffold platforms are more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface shall be used. Crossbraces shall not be used as a means of access. a) Exterior supported scaffold at south side of house: An employee was working on the 2nd platform of a 3-section-high supported scaffold. The employee did not have a means of safe access to the supported scaffold and used frames and cross braces of the scaffold as a means of access. On or about 03/28/12. Paragraphs (g)(1)(i) through (vii) of this section establish the types of fall protection to be provided to the employees on each type of scaffold. 29 CFR 1926.454(a): The employer shall have each employee who performs work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards. a) Jobsite: The employer did not provide training to an employee who worked on the 2nd platform of a 3-section-high supported scaffold. The employee could not recognize hazards associated with scaffolds, and was exposed to hazards, including but not limited to: employee exposed to fall hazard, and employee worked on scaffold platform not fully planked or decked, employee has no safe access to scaffold platforms, and scaffold legs and frames not bear on adequate firm foundation. On or about 03/28/12. NOTE: IN ADDITION TO CERTIFICATION OF ABATEMENT, THE EMPLOYER IS REQUIRED TO SUBMIT DOCUMENTATION OF ABATEMENT FOR THIS ITEM.
