Agent orange concentrate contains chemicals such as, but not limited to, potassium hydroxide. The employer did not proivde the employee with a safety goggles or face shield during mixing operations. |

On or about 1/26/12, on site, for the housekeeping employee that uses the Ultral Line Stripper to strip the floors. The employee mixes 1/2 gallon of agent orange concentrate with 1/2 gallon of water. Agent orange concentrate contains chemicals such as, but not limited to, potassium hydroxide. The employee's training did not include the explanation of the labeling system and the material data sheet, and how employees can obtain and use the appropriate hazard information. b). On or about 1/26/12, on site, for housekeeping employees that use various cleaning materials, on a daily basis, such as the 13L deodorizer, and 3M bathroom concetrate 4L. These cleaning products contains chemicals such as, but not limited to, hydrogen peroxide and polyalkoxy alcohols. The employee's training did not include the explanation of the labeling system and the material data sheet, and how employees can obtain and use the appropriate hazard information. |
