Citation text line OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck-by/crushed-by hazards from falling ceiling material: a) Basement Cafeteria - Workers were cutting away and removing plaster/lath ceiling sections along the perimeter of the basement cafeteria. The ceiling collapsed onto workers below; on, or about, 10/17/2014. Note: In addition to abatement certification, the employer is required to submit abatement documentation for this item in accordance with 29 CFR 1903.19. Among other methods, feasible and acceptable abatement methods to corrects this hazard would be for the employer to comply with: (a) ANSI/ASSE A10.6-2006, Safety and Health Program Requirements for Demolition Operations - American National Standard for Construction and Demolition Operations 5. GENERAL PROTECTION 5.1 During demolition, inspections by a competent person shall be made as the work progresses to detect hazards resulting from weakened or deteriorated floors, walls or loosened material. No worker shall be permitted to work where such hazards exist until they are corrected by shoring, bracing or other effective means. Access controls shall be established to prevent unauthorized entry into the work zone.
