The employee did not receive treatment for almost 20 minutes due to procedural delays in contacting off-site emergency medical personnel. |

| Citation text line | 29 CFR 1910.261(l)(9)(i): The nipping points of all drum winders and rewinders located on the operator's side were not guarded by either automatic or manually operated barrier guards of sufficient height to protect fully anyone working around them. The barrier guard was not interlocked with the drive mechanism to prevent operating above jog speed without the guard in place. A zero speed switch was not installed to prevent the guard from being raised while the roll is turning (a) PM 11, Cameron 450 Slitter/Rewinder: On or prior to 6/6/20, the jogging foot pedal control was not functional and all barrier guards were not interlocked with the drive mechanism, exposing employees to caught-in hazards when working inside the safety cell (i.e. pulling slab, etc.), as the machine was operated in crawl mode and the means to stop the machine in an emergency or reverse roll direction was located at an HMI control panel located outside the cell. |
