29 CFR 1910.23(c)(4): Unsecured ladders were used on surfaces that were not stable and level. a) On or about 05/02/2019, in the paint area, employee was using an A frame ladder to hook a frame to the hook on the crane to spray paint. While employee was on the ladder handling the frame he placed the crane's controller on a rung on the ladder. When the ladder became unstable due to the metal grates not being leveled underneath, he lost his balance and as he tried catching himself he accidentally pressed the button on the controller and made the load move up which crushed his right thumb causing it to be amputated.
