29 CFR 1910.138(a): The employer did not select and require employee(s) to use appropriate hand protection when employees' hands were exposed to hazards such as those from skin absorption of harmful substances; severe cuts or lacerations; severe abrasion; punctures; chemical burns; thermal burns; and harmful temperature extremes. a) Facility - On or about 6/23/15, employees work in an electroplating facility where known skin irritants, such as but not limited to, nickel, palladium, gold and silver solutions are used in the plating operations, as well as, corrosives such as hydrochloric and sodium hydroxide and fluboric acid. The employer did not select/require employees to use the appropriate hand protection such as gauntlet gloves to protect the employee from skin exposure when working with and around the solutions that are known skin irritants, which cause skin rashes, or required to wear more substantial glove to protect their hands and wrist areas. 29 CFR 1904.7(a): 1904.7(a) The employer did not record workplace injuries and illnesses that would be considered to meet the general recording criteria, and therefore to be recordable, if it results in any of the following: death, days away from work, restricted work or transfer to another job, medical treatment beyond first aid, or loss of consciousness. You must also consider a case to meet the general recording criteria if it involves a significant injury or illness diagnosed by a physician or other licensed health care professional, even if it does not result in death, days away from work, restricted work or job transfer, medical treatment beyond first aid, or loss of consciousness. a.) Facility - On or about 4/15/15, an employee sustained a chemical splash (KCN) to the R eye which resulted in irritation/chemical burn and subsequent medical treatment. The injury was not recorded on the employer's OSHA 300 log. b.) Facility - On or about 6/15/15, an employee suffered a chemical drip to the eye (from ceiling) which resulted in irritation/chemical burn and subsequent medical treatment. The injury was not recorded on the employer's OSHA 300 log. c.) Facility - On or about 6/20/15, an employee suffered a low thoracic strain, which resulted in 1 lost day. The injury was not recorded on the log. d.) Facility - On or about 4/15/15, a TEMPORARY employee was splashed in the R eye with chemicals which resulted in irritation/chemical burn and subsequent medical treatment. The injury was not recorded on the OSHA 300 log. ABATEMENT CERTIFICATION REQUIRED |
