Crossbraces were used as a means of access: a) On or about 9/21/2018, in the atrium area: Safe means of egress was not provided to all levels of the scaffold being used by employees who were painting. A built in ladder between two working levels of supported scaffolding did not extend to the upper level, was over 1.5 feet horizontally from the upper level, and there was no opening in the rail system. Employees were traversing between levels by climbing exterior bracing. Employees were exposed to falls of over 29 feet. Abatement documentation is required for this item. |
