Niagara Falls Memorial Medical Center Policy No. D-100, effective 2/15/05, latest review date 3/19, latest revision date 5/13 states "1. Respirator Selection a...."When selecting respirators for M. tuberculosis one must consider whether the respirator will be used in sterile field, such as operating rooms, or in tuberculosis (TB) isolation rooms." Policy No. D-100 does not list the respirators required for TB protection, the employee work areas, and the conditions of use. Policy No. D-100 does not indicate that N-95 respirators are required for employee use when working with suspect or COVID-19 positive patients. Citation text line 29 CFR 1926.1101(k)(9)(ii): Training was not provided prior to or at the time of initial assignment and/or at least annually thereafter for employees performing Class I through IV asbestos operations: a) S2 - On or about 6/7/17, housekeeping staff performed Class IV operations following abatement activity involving spray-on fire proofing (a Class I operation). Employees performed dusting, mopping and general cleaning in these areas. Debris found in the area was revealed to be 15% chrysotile asbestos. Employees were not provided with the required training at the time of initial assignment or annually thereafter. ABATEMENT CERTIFICATION REQUIRED
