OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that employees lowering the telescopic door were exposed to the hazard of being struck by a door panel of the telescopic door when the chains of the door panel were cut, which release the stored mechanical energy in the system. Location: a) Telescopic Door 28 on the 1st floor south side of the jobsite: On or about 06/06/17, employer did not isolate the stored mechanical energy of a counterweight that was connected to a door panel of a telescopic door, and did not rendered the counterweight inoperative. |
