The saw was connected to 120-volt ac power via an extension cord which was plugged into a wall receptacle in the building. No GFCI or assured equipment grounding program was being used. None of the four wheels on the scaffold was locked. b) On or about 03/21/13, on construction site inside former bank building, 1308 Niagara Falls Blvd., Tonawanda, NY: Employee was exposed to potential fall of approx. 5 ft.11 in. while removing overhead ductwork, while on the platform of a narrow Bil-Jax mobile metal frame scaffold.
