29 CFR 1910.22(b)(1): Where mechanical handling equipment was used, sufficient safe clearances were not allowed for aisles, at loading docks, through doorways and wherever turns or passage had to be made: a) Building D Thermoforming Lines, on or about 9/1/15: The aisle behind the thermoforming lines was not of sufficient width to allow for the safe clearance of mechanical handling equipment. The aisle width varied from approximately 6-1/2 feet to 9 feet. On 9/1/15 Toyota Forklift #25 was being operated behind the thermoforming lines when an employee working at the 311 unwind stand stepped back into the aisle and then forklift. The employee was struck-by the forklift which resulted in multiple broken bones to their right leg. |
