The employees were working from a height of approximately 16 feet 8 inches. According to the manufacturer's instructions, the maximum stackable levels for this scaffold is up to 2 levels. The scaffold was not constructed by a qualified person. |

The two employees were working from a height of approximately 16 feet 8 inches. The base width dimension on the scaffold was 28 inches. The height to base width ratio for the scaffold was approximately a 7:1 ratio. The scaffold was not restrained from tipping by guying, tying, bracing, or equivalent means. |
