On or about August 8, 2012 at 51-18 Grand Avenue, 2nd Floor, Maspeth, NY 11378 A portable light source not specifcally approved for locations containing deposits of readily ignitiable/flammable residue or explosive vapors was observed plugged into an electrical outlet approximately six feet away from a spray painting operation exposing employees to spark/flame/fire hazards. |
