Citation text line 29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii) (a) 46 Carleton Avenue, Central Islip, NY (Worksite), southwest corner of building  Employees worked from a platform that was located on the third level of the fabricated frame scaffold at a height of 14' 11" above the ground below. The platform was comprised of two planks, measuring approximately 18-1/2" wide, placed along the centerline of the 39" wide opening between the uprights. There were two planks missing, which resulted in openings, approximately 10-1/2" wide, between each side of the platform and uprights on, or about June 30, 2014. (b) Worksite, southwest corner of building  An employee worked from a platform that was located on the third level of the fabricated frame scaffold at a height of 14' 11" above the ground below.
