29 CFR 1926.34(a): A lock or fastening to prevent free escape from the inside of any building was installed. Building or structure exits were not so arranged and maintained as to provide free and unobstructed egress from all parts of the building or structure at all times when it was occupied. a) Jobsite entrance, 1st floor: A lock was installed on the entrance door which prevented free escape from the inside of the building. Employees were locked inside the site. Demolition debris was stored in the building such that it obstructed egress from the 1st floor when employees worked on site. On or about 07/29/16. |

29 CFR 1926.855(c): Safe walkways, not less than 18 inches wide, formed of planks not less than 2 inches thick if wood, or of equivalent strength if metal, were not provided and used by workmen when necessary to enable them to reach any point without walking upon exposed beams. a) 2nd floor and 1st floor: Employees walked on exposed floor beams while doing demolition work. Planks were not provided to them for use as safe walkways. Employees were exposed to falling up to 20 feet through between floor beams. On or about 07/29/16. |
