Citation text line 29 CFR 1926.404(b)(1)(ii): On a construction site, where an assured equipment grounding program was not utilized, all 120-volt, single-phase, 15 and 20 ampere receptacle outlets which were not a part of the permanent wiring of the building or structure and which are in use by employees did not have approved ground fault circuit interrupers for personal protection. (a) Yates village building G, during roof tear-off and repair: On or about 5/11/16, extension cord used to power a porter-cable pancake compressor was not used with GFCI protection, exposing employees of subcontractor Five Star Solutions Corp. to a potential shock hazard. Cord was connected to a 120V receptacle inside the mechanical room of the building. (b) Yates village building G, during roof tear-off and repair: On or about 5/11/16, extension cord used to power a circular saw on the roof deck was not used with GFCI protection, exposing employees of subcontractor Five Star Solutions Corp. to a potential shock hazard. Cord was connected to a 120V receptacle inside a vacant apartment.

This increases the probability of anchor point failure in a fall arrest or fall restraint event. Employees were exposed to a fall hazard of 18 to 20' to ground level.
