The employee was not wearing eye protection; or about 3/3/2016. Note: Because abatement of this violation is already documented in the case file, the employer need not submit certification or documentation of abatement for this violation as normally required by CFR 1903.19. |

The employee was wearing a harness, with a lanyard but was not attached to a lifeline; on or about 3/3/2016.
