Citation text line 29 CFR 1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns. a) Behind the house, south side: Employees were operating an overhead hoist motor to hoist a single-point suspension scaffold and a section of metal ventilation duct. An employee who was working from the scaffold platform was not protected by a protective helmet while operating the hoist. Both the scaffold and the duct were connected to the hoist and anchored to a roof brick chimney, which was overloaded and collapsed into brick pieces in an accident. The duct was approximately 20 feet high from the ground, and an employee worked from the scaffold at more than 10 feet above ground.
