29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: a) On or about 5/08/17, on the roof at the front and left sides of a house at 7201 Pendale Drive, Pendleton, NY: Three employees engaged in roofing tear-off work on a steep roof were exposed to a fall of approx. 9 to 15 ft. from various locations on the roof. No fall protection system was utilized. Pitch of the roof was approx. 10 in 12. NO ABATEMENT CERTIFICATION REQUIRED |

29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury: a) On or about 5/08/17, on a roofing construction site at a house at 7201 Pendale Drive, Pendleton, NY: Employees engaged in construction work including roofing & clean-up were not trained to recognize potential hazards on this type of jobsite, including but not limited to falls from roofs, ladders, skylights, stairs & scaffolds, electrical hazards, unguarded tools/machines, fire hazards, and the need for personal protective equipment to protect them from various hazards. They also were not trained in the applicable OSHA regulations to control or eliminate these hazards. ABATEMENT CERTIFICATION REQUIRED |

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: a) On or about 5/08/17, at the front of a house at 7201 Pendale Drive, Pendleton, NY: Employees used an eight foot stepladder to access and exit a narrow aluminum pik on a ladder jack scaffold that was approx. 8 ft. above the ground. The side rails of that ladder only extended approx. 7 inches above top of the pik. Additionally, an employee used the top step of the stepladder when climbing and descending the ladder. NO ABATEMENT CERTIFICATION REQUIRED |
