29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: a) On or about 03/25/14, on the front side of a 2-story residential house at 78 Gardenwood Ln., Tonawanda, NY: Four employees engaged in roofing work were exposed to a potential fall of approx. 18 ft. to the ground when an extension ladder was used to access the roof from the ground. The siderails of the ladder only extended approx. 16 inches above the roof edge. b) On or about 03/25/14, on the Back side of a 2-story residential house at 78 Gardenwood Ln., Tonawanda, NY: Employees engaged in roofing work were exposed to a potential fall of approx. 11 ft. to the ground when an extension ladder was used to access the roof from the ground.
