Citation text line 29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: a) On or about 06/03/16, on the south side of a house at 881 Richmond Ave., Buffalo, NY: A ladder jack scaffold was erected on the south side of the house. Employees used an extension ladder to access the pik of the scaffold. The employees were exposed to a fall of approx. 19 ft. to the ground because the siderails of an extension ladder used to access the scaffold from the ground only extended approx. 30 inches above the pik. b) On or about 06/03/16, on the north side of a house at 881 Richmond Ave., Buffalo, NY: An extension ladder was erected at the northwest corner of the roof. Employees used the extension ladder to access the north roof on the house. The employees were exposed to a fall of approx. 21 ft. to the ground because the right siderail of the extension ladder only extended approx. 6 inches above the edge of the roof, and the left siderail of the extension ladder only extended approx. 20 inches above the edge of the roof. NO ABATEMENT DOCUMENTATION REQUIRED
