Citation text line 29 CFR 1926.454(a): The employer shall have each employee who performs work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards. a) Jobsite: The employer did not provide training to employees who erected and worked from a ladder jack scaffold. Employees could not recognize hazards associated with the scaffold, and were exposed to hazards, including but not limited to: falling more than 10 feet from the scaffold platform to ground. On or about 11/19/13. NOTE: IN ADDITION TO CERTIFICATION OF ABATEMENT, THE EMPLOYER IS REQUIRED TO SUBMIT DOCUMENTATION OF ABATEMENT FOR THIS ITEM. Citation text line 29 CFR 1926.452(k)(3): The ladder jack shall be so designed and constructed that it will bear on the side rails and ladder rungs or on the ladder rungs alone. If bearing on rungs only, the bearing area shall include a length of at least 10 inches (25.4 cm) on each rung. a) In front of house: Three employees were working on a platform of a ladder jack scaffold and the ladder jack did not bear on the side rails of both ladders being used to suport the platform. One extension ladder was erected with the platform support on the interior portion of the ladder bearing on the wall of the buidling. Employees were exposed to falling approximately 14 feet. On or about 11/19/13. Note: Because abatement of this violation is already documented in the inspection case file, the employer need not submit certification of abatement for this violation as normally required by 29 CFR 1903.19.
