Citation text line 29 CFR 1926.451(e)(1): When scaffold platforms are more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface shall be used. Cross braces shall not be used as a means of access. a) Exterior supported scaffold at the west building fa�ade: Four employees were working on platforms of a 3-story-high 5-column-long supported scaffold. Employees did not have a means of safe access to the supported scaffolds, and used frames and cross braces of the scaffolds as a means of access. On or about 12/08/12. Employees could not recognize hazards associated with supported scaffold, and were exposed to hazards, including but not limited to: falling up to 30 feet from scaffold to ground without fall protection, climbing frames and cross braces of supported scaffold as a means of access, and working on scaffold platforms not fully planked or decked.
