Crossbraces were used as a means of access: a) Worksite, north side of building- Employees climbed the frames and crossbracing of a fabricated frame scaffold to access the 12' and 18' levels; on or about 06/29/12. Note: The employer is required to submit abatement certification for this item in accordance with 29 CFR 1903.19.

The scaffold had only four planks installed. Two planks were missing, leaving an opening 18" wide; on or about 06/29/12.
