Citation text line 29 CFR 1926.502(d)(8): Horizontal lifelines were not designed, installed, and used under the supervision of a qualified person, as part of a complete personal fall arrest system, which maintained a safety factor of at least two. a) Southwest side of jobsite, 2nd floor metal decking: Three employees installed and used two horizontal lifelines as anchors for fall protection during metal decking installation. The horizontal lifelines were not designed, installed, and used under the supervision of a qualified person. Two employees who were tied off to the same horizontal lifeline with retractable lanyards; they fell 20 feet and landed on lower level in an accident. The horizontal lifeline deflected and the fall arresting mechanism of the retractable lanyards did not trigger to save employees from fall. On or about 03/18/13.
