Citation text line OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck-by/crushed-by hazards due to equipment operators not being trained to inspect or operate equipment: a) Worksite, North - No inspection or operation training was provided for an employee who was operating a Takeuchi TL 240 Compact Track Loader. A bucket attachment fell from the track loader and struck another employee on, or about, 01/23/2014. Note: In addition to abatement certification, the employer is required to submit abatement documentation for this item in accordance with 29 CFR 1903.19. ABATEMENT NOTE: Recommended Practice: Read and understand instructions in the Operator's Manual, Takeuchi TL 240 Compact Track Loader. 1) Among other methods, one feasible and acceptable abatement method to correct these hazards is to provide employees training to inspect and operate the Takeuchi TL 240 Compact Track Loader. 2) Visually confirm that quick-hitch latch pins are engaged. 3) Conduct walk-around inspection of vehicle as prescribed by the manufacturer. 4) Travel with bucket cylinder fully retracted and the bucket at a height of approximately 12 inches from the ground.
