Citation text line 29 CFR 1926.651(j)(2): Protection was not provided by placing and keeping excavated or other materials or equipment at least 2 feet (.61m) from the edge of excavations, or by the use of retaining devices that were sufficient to prevent materials or equipment from falling or rolling into excavations, or by a combination of both if necessary. a) Building footing trench - On or about 18 May, 2012 - Employees were in the bottom of an excavation tying rebar in the footer form with concrete and earth materials along the edge of the excavation above where they were working. Material should have been stored a minimum of 2 feet from the edge of the excavation. Employees were exposed to a struck by hazard.

Citation text line 29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees: a) Building foundation excavation - On or about 18 May, 2012 - Employees were working in a rectagular trench that was 80 feet long on two sides and 30 feet wide on two sides and had a trench width of 5 feet installing a footer form. Employees only had 2 means of egress out of the excavation, a ladder and a sloped entrance in the event of a cave in. Employees were exposed to a struck by or crushing hazard.
