Location: 101 Sixth Ave, New York, NY 10019 In front of Building. a) Employees assembled a baker scaffold that only had the outrigger braces at one side. The baker scaffold was free standing and when an employee applied eccentric loads (extension ladder weight and body weight) to the baker scaffold the scaffold tipped over. An employee fell approximately 16 ft to the ground below; on or about 9/27/13. |
