29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, or enclosures was not permanent and continuous. a) 2nd floor, west side: Employees used an electric powered screwdriver to do framing work. The screwdriver was connected to an extension cord, which was routed to another extension cord that is routed to an electric power source on the 1st floor. The immediate connected extension cord was missing a ground pin, and second connected extension cord did not have its ground pin plugged in. On or about 05/06/14.
