Citation text line 29 CFR 1926.1053(b)(6): Unsecured ladder(s) were used on surfaces that were not stable and level: a) Front of house: An employee was utilizing a 40-foot high aluminum extension ladder to carry a sheet of asphalt paper to the ridge of the 2nd floor roof. The ladder was not secured to prevent accidental displacement and slid backwards when the employee climbed to the top of the ladder. The employee fell off the ladder and onto the roof and tumbled down to ground. On or about 07/31/12.
