The floor at the entry/exit was removed exposing the floor joists and basement below. Employees were utilizing this area as a means of entry/exit into the project. |

On or about October 30, 2014 and October 31, 2014 Location: 155 West 126th Street, New York, NY every stair well throughout the project. Employees were traversing throughout the project using stairwells and were exposed to trip and fall hazards because the stair landing pans were not filled in with a solid material and the material used to fill stair treads had been worn down below the top lip of the tread pan. |

On or about 12/07/12, scaffolds at 29-20 Healy Ave. Far Rockaway, NY Employees were ascending/descending a scaffold by climbing the frame and were in danger of falling approximately 24 ft to the ground.
