The employee was not protected from falling; on or about 07/09/13. b) Worksite, Northeast side roof - An employee installing sheathing approximately 18 feet above the ground on an approximate 8-12 pitch roof had a harness and a lanyard on but was not attached to the lifeline. The employee was not protected from falling; on or about 07/09/13. Note: Because abatement of this violation is already documented in the case file, the employer need not submit certification or documentation of abatement for this violation as normally required by CFR 1903.19. Employee was exposed to a fall of approximately 25ft to the ground below; on or about 10/16/12. b) Worksite, Interior - Employees were accessing the second floor of a residential home by walkin on a 2x10in plank. The employees were not provided with fall protection and were exposed to a fall of approximately 9ft to the ground below; on or about 10/16/12.
