The employee was not protected from falling; on or about 07/09/13. b) Worksite, Northeast side roof - An employee installing sheathing approximately 18 feet above the ground on an approximate 8-12 pitch roof had a harness and a lanyard on but was not attached to the lifeline. The employee was not protected from falling; on or about 07/09/13. Employee was exposed to a fall of approximately 25ft to the ground below; on or about 10/16/12. b) Worksite, Interior - Employees were accessing the second floor of a residential home by walkin on a 2x10in plank. The employees were not provided with fall protection and were exposed to a fall of approximately 9ft to the ground below; on or about 10/16/12. Note: The employer is required to submit abatement certification for this item in accordance with 29 CFR 1903.19.
