Crossbraces were used as a means of access. Location: a) Scaffold exterior right side of the building of the jobsite: On or about August 1st, 2023, Employees were working from a one-tier tubular welded framed suspended scaffold and had no safe access to and from the working platform. Employees used frames and cross braces of the scaffolds as a means of access. |

Location: a) Exterior wall on the right side of the building of the jobsite: On or about August 1st, 2023, an employee stood on the 1st tier of the scaffold, approximately 6 feet above the 1st-floor level, to perform masonry bricklaying work on the 1st-floor exterior wall of the building. The working platform was not fully planked or decked between the front uprights and the guardrail supports. |
