29 CFR 1910.134(k)(6): The employer did not provide the basic advisory information on respirators, as presented in Appendix D of 29 CFR 1910.134, in written or oral format to employees who wear respirators when such use was not required by the employer: a) On or about June 11, 2015 - at job site at 30-30 47th Avenue, Long Island City, NY 11101, 3rd Floor, Building 4 section Employees working on scissorlifts, including some operating powered demolition hammers to remove cork insulation from ceilings, and employees working below the lifts at floor level sweeping and cleaning up debris, who were wearing N95 respirators, had not been provided with the basic advisory information on respirator use presented in 29 CFR 1910.134 Appendix D. This lack of information was reflected in the incorrect donning of the respirators by several employees, and the unsanitary condition of the respirators which in some cases were heavily soiled on the inner surface of the mask. |
