29 CFR 1926.1053(a)(1)(ii): Non-self-supporting portable ladder(s) were not capable of supporting without failure, at least four times the maximum intended load: a) Jobsite in Altamont, NY front of building - On or about April 23, 2014, a Werner aluminum extension ladder (Type III, 200lb load capacity, Model D1116-2) was used to access a roof exposing employees to a fall hazard of approximately 10 feet to ground level. An employee carrying tools and supplies exceed the load capacity of the ladder. b) Jobsite in Altamont, NY rear of building - On or about April 23, 2014, a Werner aluminum extension ladder (Type III, 200lb load capacity, Model D1116-2) was used to access a roof exposing employees to a fall hazard of approximately 10 feet to ground level.
