Citation text line 29 CFR 1926.451(b)(5)(ii): The end of a platform greater than 10 feet in length extended over its support more than 18 inches and the platform was not designed and installed so that the cantilevered portion of the platform is able to support employees without tipping, or has guardrails which block employee access to the cantilivered end. (a) Worksite, west side of the residential home, ladder jack scaffold - Employees were installing 2" x 2" drip edge along the west edge of the first story roof from a ladder jack scaffold. The Werner 24' aluminum scaffold stage, Model # 2424, used to create the work platform was extended 8' beyond the south support and was not secured to prevent it from tipping, which exposed the employees to being struck-by scaffold components and a fall of 8' to the ground below on, or about July 12, 2013. Note:The employer is required to submit abatement certification for this violation in accordance with 29 CFR 1903.19.

(b) Worksite, north side of the residential home, first story roof - Employees were engaged in re-roofing work on the first story of a residential home. The employees were working at a height of 11' on the 4:12 pitch roof, without fall protection, which exposed the employees to a fall of 11' to the ground below on, or about July 12, 2013. Note: In addition to abatement certification, the employer is required to submit abatement documentation for this violation in accordance with 29 CFR 1903.19.
