The Werner 24' aluminum scaffold stage, Model # 2424, used to create the work platform was extended 8' beyond the south support and was not secured to prevent it from tipping, which exposed the employees to being struck-by scaffold components and a fall of 8' to the ground below on, or about July 12, 2013.
