29 CFR 1910.178(a)(5): The powered industrial truck was equipped with front-end attachments other than factory installed attachments, however the employer did not request that the truck be marked to identify the attachments and show the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered: a) On or about 02/06/19 in and around the yard area and truck unloading; employees were exposed to being struck by wooden crates of glass weighing between 500 - 1,000 pounds when powered industrial trucks (PIT) used boom extensions, such as but not limited to the Caldwell brand fork lift boom (model PB 60 - serial 94217), to lift these wooden crates off of the bed of tractor trailers and name plate on the Toyota PIT ( serial number 10484) did not have the information related to use of the lift boom according to the manufacturer's instructions or approval.
