29 CFR 1926.451(f)(7): Scaffolds were not erected, moved, dismantled or altered under the supervision and direction of a competent person qualified in scaffold erection, moving, dismantling or alteration. Such activities were not performed by experienced and trained employees selected for such work by the competent person: a) 1111Victory Blvd., Staten Island, NY 10301. The employee was exposed to fall hazards and electrocution hazards during the erection of Alum-A-Pole pump jack scaffold when the owner, i.e. Competent Person did not or could not recognize the hazards in that: Instance #1 The employer allowed his employee to work on and from the Alum-A-Scaff platform in excess of 11 feet above the ground with no fall protection exposing him to fall hazards. Instance #2 The employer allowed his employee to erect the Alum-A-Pole scaffold within three feet of energized power lines exposing them both to electrocution hazards. Incident occurred on or about 09/03/15. |
